Policy No: 627 Policy Contact: Vice President for Business Operations Policy Title: PROCEDURES FOR DOCUMENTING INFORMATION RELATING TO POTENTIAL GENERAL LIABILITY CLAIMS East Central Community College (ECCC) provides general liability insurance protection for the purpose of reducing ECCC’s risk of financial exposure in the event of personal or property damage as a result of negligence on the part of ECCC. Should a potential claim situation arise, the following information will be collected generally through official incident report filed with the College Police Department.

Personal Information 1. Name/address/telephone number of claimant 2. Name/address/telephone number of claimant’s personal representative

3. Date of birth 4. Marital status

Basis of Claim

1. Date and day of the accident 2. Time (A.M. or P.M.)

3. Statement in detail of the known facts and circumstances attending the damage, injury, or death; identification of the persons and property involved; the place of occurrence; and the cause thereof Property Damage 1. Name/address/telephone number of the owner, if other than claimant 2. Description of the property, the nature and extent of the damage, and location where the property may be inspected 3. An accident report from law enforcement officials if a motor vehicle is involved

Personal Injury/Wrongful Death 1. Nature and extent of each injury or cause of death Witnesses 1. Names/addresses/telephone numbers of all witnesses

Insurance 1. Names/addresses/policy numbers of all insurance companies 2. Determination as to whether a claim has been filed 3. Action taken by insurance carrier 4. Notification of the college’s insurance company of a potential claim
